Remarks: 

Samples involved with employment situations must be accompanied by a chain of custody form and be sealed at the time of delivery to the Laboratory. For employment situations, the sample must be collected according to the employment urine drug screen procedures with checks for temperature and color and also proper patient and collection protocols. The client must be identified by a photographic identification card, and the sample must be sealed in the client's presence and initialed by the client.

Unacceptable Conditions: 

Samples that are ambiguously labeled, unsealed, or fail the temperature or chemical checks will be rejected and must be recollected.
