Send specimen Monday through Thursday only.

Specimen Type: Muscle biopsy, preserved

Container/Tube: Sterile vial with formalin and/or glutaraldehyde with no more than 30 mL liquid per vial

Specimen Volume: Entire collection

Additional Information: 

1. Specimen source is required.

2. Ship in a box with absorbent material and affix a “UN3373 Diagnostic Specimen” label.

3. Ship via FedEx Priority Overnight, marking the bill for 3rd party (268-54-8564), to the following address:

3. Call with FedEx tracking number before shipping.

Forms: UIDL Anatomic Pathology Consult Requisition-Include patient history. Indicate “Fax results to Pathology Department at 712-279-7034” on order.

Container/Tube: Sterile vial

1. Specimen source is required.

2. Ship frozen with 6 to 8 lbs. of dry ice with “UN3373 Diagnostic Specimen” label, “UN1835 Dry Ice” label, and “Hazard Class 9” label affixed to box.

3. Ship via FedEx Priority Overnight, marking the bill for 3rd party (268-54-8564), to the following address:
